  • Conflict among narrative, table, or chart.
  • Chart label, axis, or category error.
  • Translation changing the meaning.
  • Broken link or file.
  • Inaccurate methodology, date, or description.

  • Possible exposure of a person or sensitive location.
  • Misleading external use of a Center finding.

Priority

Priority is given to participant-safety, personal-data, major-conclusion, and multi-format or language issues. Disagreement with interpretation is not necessarily an error, but may still merit review.

Review

The report is checked against source material, tables, code, and versions. The outcome may be no error, a minor correction, a material notice, a new version, or withdrawal under the Corrections Policy.

Error report form

Do not send additional personal information to prove a privacy issue. Describe the location and request a secure channel if needed.

Suggested confirmation

Thank you for raising the issue. It will be reviewed against the material, version, and available sources. Receipt does not establish that an error occurred, but the report will be registered and assessed.
